Transitions At Home is currently seeking to hire a full-time Physical Therapist.

What does a Transitions at Home Physical Therapist do?
An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
Hours:
- 32-40 hours per week 8:00 am - 4:30 pm
- Monday through Friday
- No Holidays

Qualifications /
Job Requirements:
- Active Wisconsin Physical Therapist License
- Two (2) years’ experience preferred
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
