SummaryResponsible for the use of advanced nursing processes to identify high‑risk, acutely ill, and patients with chronic disease. The role of this position is to improve health outcomes through a population‑health based platform. The position is accountable for assessing, planning, educating, monitoring, evaluating, and coordinating services for patient populations and their families, to foster quality, continuity, and appropriate utilization of health‑care resources throughout the continuum of care.
Facilitates the coordinated utilization of resources to maximize health outcomes, patient/family satisfaction, and financial outcomes. Assists patients, families, and caregivers in securing necessary medications, equipment, and community resources to facilitate health and overall wellness. As a member of a multidisciplinary team, consults with other health‑care team members to coordinate the services of preventive care and chronic disease management.

Minimum Qualifications- Formal
Education:
Bachelor of Science in Nursing (BSN). Applicants with an associate’s degree will be considered but must obtain a BSN within three (3) years of employment. Graduate of a program of professional nursing that is accredited by the Commission on Collegiate Nursing Education (CCNE) or Accreditation Commission for Education in Nursing (ACEN). - Current license to practice registered nursing in the State of Ohio.
- Experience & Training:
Three (3) years clinical experience in care coordination; strong clinical experience with diabetic populations and other chronic diseases; strong clinical knowledge in promoting healthy lifestyles and preventive screenings; one (1) year of Care Management preferred; knowledge of Epic electronic medical records software preferred; additional related previous experience and/or preparation required for specialty care management. - Other Skills, Competencies and
Qualifications:
Demonstrates the ability to manage time and promote departmental and professional growth; able to use electronic software applications related to care management activities (e.g. Healthy Planet, payer portals to retrieve population health data, CMS portal data retrieval); act as a subject matter expert for clinical staff education materials and other resources; implement care management activities in their assigned office sites; complete and attend all required educational offerings;
participate in continual improvement of care management processes; knowledge of current practices in population health; knowledge of quality improvement/performance improvement tools; proficiency with computer software programs, e.g. MS Outlook, Word, Excel, and PowerPoint; ability to set goals independently and prioritize work to balance competing deadlines in a professional manner; excellent customer service skills; excellent written and verbal communication skills; ability to manage multiple tasks. - Level of Physical Demands:
Sedentary; may exert up to ten pounds of force occasionally and/or a negligible amount of force frequently.
Salary range: $37.40/hr – $56.11/hr (base salary exclusive of any bonuses or differentials). The range listed is the full base salary range. Hiring at the maximum of the range would not be typical.
