What you will do
  • Clinical Assessment: Undertake comprehensive clinical assessments for individuals accessing substance misuse services.
  • Care Delivery: Deliver evidence-based nursing interventions and tailored treatment plans.
  • Health Monitoring: Perform physical health checks including BBV screening, wound care, and health monitoring.
  • Medication Management: Administer and monitor prescribed medications in line with clinical protocols.
  • Treatment Support: Support opioid substitution therapy and other pharmacological interventions.
  • Outreach Engagement: Deliver outreach services to individuals who are homeless or reluctant to engage with services.
  • Relationship Building: Establish strong therapeutic relationships to encourage engagement in recovery programmes.
  • Harm Reduction: Provide harm reduction advice, including overdose prevention and safer substance use practices.
  • Safeguarding: Identify safeguarding concerns and take appropriate action in line with procedures.
  • Multi-Agency Working: Collaborate with healthcare providers, housing, probation, and social care teams.
  • Service Improvement: Contribute to audits, governance, and quality improvement initiatives.
