Are you committed to delivering compassionate, person‑centred care that empowers individuals to remain independent? Join our dynamic Neighbourhood Rehabilitation Team as a Rehabilitation Support Worker (RSW) & make a real difference in your community.
You will work under the guidance of registered healthcare professionals to deliver high quality, person‑centred care on the Intermediate Care pathway. You will work both independently & collaboratively supporting individuals in their own homes & care homes.
Deliver delegated care tasks including personal care & medication support. Promote independence through mobility practice, encouraging participation in exercise programmes/therapy plans & assisting with meal preparation & daily living skills.
Desirable to work in the NHS, be motivated & flexible, and work a shift pattern including weekends & bank holidays.
This is a community role; a current driving licence & access to a vehicle to undertake travel in connection with the duties of the post is essential.
Visa sponsorship considerations: Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000. For Band 3 roles, only full‑time positions at the top of the pay scale meet this threshold. Therefore, sponsorship may not be possible depending on salary increment placement and working hours.
Main duties of the postAs a Rehabilitation Support Worker you will:
- Provide high standards of safe & compassionate care that maximises patients’ rehabilitation potential.
- Work in close liaison with the MDT, maintaining excellent communication and time‑management skills.
- Assist with initial assessment & care plan development/implementation.
- Participate in training, education & clinical supervision to develop skills & knowledge and apply continuous reflection on own practice.
- Work independently and as part of a team.
- Demonstrate strong communication and observational skills.
- Maintain accurate clinical records.
- Support safe use of equipment such as hoists, walking aids & pressure cushions.
- Provide information to support your application relating to your skills & personal qualities, telling us how you meet the person specification criteria providing examples.

Job responsibilities (summary)The post holder will:
- Perform delegated routine interventions and care tasks under the direction of registered healthcare professionals.
- Support appointments with service users in their homes and the community (including care homes), mainly working alone under direction or with other team members.
- Assist with personal care tasks including bathing, toileting, dressing and administering pre‑dispensed medication.
- Provide assistance with meals and drinks, keeping a record of all activities.
- Support service users in following therapy care plans to promote rehabilitation, including bed exercises, stair practice & balance exercises.
