Job Overview

Are you committed to delivering compassionate, person‑centred care that empowers individuals to remain independent? Join our dynamic Neighbourhood Rehabilitation Team as a Rehabilitation Support Worker (RSW) & make a real difference in your community.

You’ll work under the guidance of registered healthcare professionals to deliver high quality, person‑centred care on the Intermediate Care pathway. You’ll work both independently & collaboratively supporting individuals in their own homes & care homes.

You will deliver delegated care tasks including personal care & medication support. Promoting independence through mobility practice, encouraging participation in exercise programmes/therapy plans & assisting with meal preparation & daily living skills.

You’ll have a desire to work in the NHS, be motivated & flexible, & work a shift pattern including weekends & bank holidays.

This is a community role – a current driving licence & access to a vehicle to undertake travel in connection with the duties of the post is essential.

Responsibilities
  • Provide high standards of safe & compassionate care that maximises patients rehabilitation potential.
  • Work in close liaison with the MDT; excellent communication & time management skills are essential.
  • Assist in initial assessment & development/implementation of care plans.
  • Participate in training, education, & clinical supervision to develop skills & knowledge & apply continuous reflection on own practice.
  • Provide information to support your application relating to your skills & personal qualities; tell us how you meet the person specification criteria providing examples.

  • Maintain accurate clinical records.
  • Support safe use of equipment such as hoists, walking aids & pressure cushions.

Visa Sponsorship

Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000, as required by UK Visas and Immigration. For Band 3 roles, only full‑time positions at the top of the pay scale meet this threshold. Therefore, sponsorship may not be possible depending on salary increment placement and working hours.
