A community-focused NHS Trust in Barnsley is looking for a passionate Therapy Liaison to enhance patient journeys within our neighbourhood rehabilitation service. The successful candidate will support patients and their families during discharge, coordinate practical support, and be a key link in the multidisciplinary team. Strong communication skills, empathy, and a NVQ Level 3 qualification in health-related fields are essential.

This position is part-time at 30 hours per week, with flexibility required for working hours.
