Overview
This role involves coaching and motivating patients in managing their own health and wellbeing to live independently and improve their health outcomes. The post holder will work with individual and/or groups of patients to deliver sessions on healthy eating, physical activity and health and well-being to meet personalised care plans. The role works within their practice's Multi Disciplinary Team; induction and training is given to help candidates integrate into Primary Care.
This is a part-time role. Days and times to be discussed at interview stage. The role is based at various locations across Barnsley.
- Provide coaching and motivation to patients to manage their health and wellbeing and meet personalised care plans.
- Deliver sessions on healthy eating, physical activity and health and well-being to individuals or groups.
- Work within the practice’s Multi Disciplinary Team and collaborate with clinical and non-clinical colleagues across the PCN to achieve optimal patient outcomes.
- Support self-management education, peer support, case management and facilitate group consultations as required.
- Utilise coaching skills to support people with lower levels of patient activation to develop knowledge, skills and confidence to manage their health and wellbeing, and to access community support offers.
- Maintain a non-judgemental approach with strong communication and negotiation skills; support personal choice and positive risk-taking while ensuring patient accountability for actions and decisions.
- Contribute to improving the quality of care and operational efficiencies within primary care.
Part-time role. Days and times to be discussed at interview stage. The role is based at various locations across Barnsley. Induction and training will be provided to help integrate into Primary Care. You will work in partnership with clinical and non-clinical colleagues across the PCN to ensure the role delivers the best possible outcomes for patients.
