Specialist Nurse – Health Inclusion Team

An exciting opportunity has become available for a Specialist Nurse with the Health Inclusion Team for 28.5hrs. We are looking for a strong team player, who can work on their own initiative and provide a person‑centred approach to patients’ independence and well‑being. The ideal candidate will bring enthusiasm, dedication and resilience, with the ability to remain positive and adaptable when faced with challenges.
A genuine passion for supporting others and achieving excellent outcomes is essential.
- To support UK new entrants, register with their preferred choice of local GP
- TB screening and blood‑borne virus screening, adults and children
- Referrals and signposting to other services
- Attend the multi‑agency drop‑in at Hope House and community events
- Self‑motivated and confident in analysing issues, implementing effective solutions independently
To work with the HIT Lead Nurse and other team members to provide high‑quality holistic health needs assessments, which is culturally sensitive to ethnic minorities and other marginalised client groups e.g. asylum seekers, homeless, gypsy and travellers residing within the borough of Barnsley.
To offer health assessments/screening to clients within this group; incorporating TB and other communicable disease screening as appropriate.
Signposting clients to appropriate services.
Clients will be supported to access and integrate with mainstream health and other services.
To act as a resource for health and other statutory/voluntary agencies.
To promote awareness and good practice in the delivery of health care for the client group.
Key Result Areas (Additional)- Responsible for the management of a transient generic caseload.
- Responsible for the management of other team members.
- Participate with the training of other health professionals, including GP practices and outside agencies.
- Act as team co‑ordinator and take on some duties relating to the Lead Nurse role, as required or delegated.
To work as a member of the Health Inclusion Team, developing partnership working and providing specialist knowledge in this field.
Organise and manage corporate workload, delegating and providing assistance to others as required.
Manage a defined client group within the team.
Using a holistic approach, provide a baseline and initial assessment for clients, in accordance with nursing protocols, taking into account cultural background.
Provide tuberculosis screening, treatment and contact tracing in line with Barnsley and National TB screening policy and guidelines.
Identify other screening or therapeutic interventions, signposting to appropriate services as required, ensuring clients’ health needs are addressed.
Interventions to be undertaken in accordance with procedures, policies and protocols, incorporating matters of a sensitive nature, including previous trauma or torture, imprisonment, rape, HIV, sexual or environmental health issues, safeguarding children, vulnerable adults, mental health issues, communicable and notifiable disease including TB. Refer to other services as appropriate.
Facilitate access to health services, including assistance with registration at GP practices as required.
Undertake any health promotion activities relevant to this client group, in association with the Health Protection Agency, Public Health Department (e.g. flu vaccinations).
Act as a specialist resource to provide education and advice on health provision in relation to this client group, both locally and nationally, to all agencies involved; including other health care professionals and students.
Utilise interpretation services effectively in accordance with policy; report any inefficiencies.
Adhere to and contribute to the development and review of policies, procedures and standards relating to the service, using an evidence‑based approach.
Work within and contribute to the clinical governance framework to maintain consistently high standards of care, working from an evidence base or current best practice.
