Position: Macmillan Clinical Nurse Specialist (CNS) in Palliative Care

Responsibilities

  • Provide supportive palliative care across Barnsley neighbourhoods to enhance quality of life for patients/carers with any life limiting illness requiring palliative care, including complex physical, psychological & emotional needs.
  • Assess, develop and implement specialist palliative care for patients and families, monitoring and evaluating the effectiveness to enable patients to be maintained in the environment of their choice.
  • Work closely with neighbourhood teams, primary care and adult social care partners to ensure seamless support for individuals with complex needs.
  • Be equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment and act in accordance with the NMC Code Professional standards of practice.
  • Have a key role in training, education and staff development, actively participate in regular supervision and appraisals contributing to clinical leadership and facilitating a culture of learning and reflective practice.
  • Have high level communication and interpersonal skills.
  • Work at weekends and bank holidays.
  • Possess extensive knowledge of palliative medication to support the complex symptom management needs of patients.
Qualifications
  • Ability to manage a caseload of people with specialist palliative care needs.
  • Excellent working knowledge of pain and symptom management, and of managing complex emotional and psychological needs.
  • Ability to provide highly specialised advice or guidance to members of the wider health and social care team.
  • Ability to design and deliver palliative care education.
  • Ability to work as an effective team member.
