Job Summary

An exciting opportunity has become available for a Specialist Nurse with the Health Inclusion Team for 28.5 hrs. The role requires a strong team player who works independently and provides a person‑centred approach to patients' independence and well‑being.   • Support UK new entrants, register with their preferred local GP
  • TB screening and Bloodborne Virus screening for adults and children
  • Referral and signposting to other services
  • Attend the multi‑agency drop‑in at Hope House and community events
  • Analyse issues and implement effective, independent solutions
Responsibilities
  • Work with the HIT Lead Nurse and team to provide holistic health needs assessments that are culturally sensitive to ethnic minorities and other marginalised client groups.
  • Offer health assessments and screening, incorporating TB and other communicable disease screening as appropriate.
  • Signpost clients to appropriate services and support them to access and integrate with mainstream health and other services.
  • Act as a resource for health and other statutory/voluntary agencies and promote awareness and good practice in health care delivery.
  • Manage a transient generic caseload and, where required, other team members.
  • Participate in training of other health professionals, including GP practices and outside agencies.
  • Coordinate the team and take on duties relating to the Lead Nurse role as required.
  • Provide TB screening, treatment and contact tracing in line with local and national policy.
  • Identify other screening or therapeutic interventions and signpost to appropriate services, ensuring client health needs are addressed.
  • Facilitate access to health services, including GP registration assistance.
  • Undertake health promotion activities, e.g., flu vaccinations.
  • Provide education and advice on health provision to agencies locally and nationally.
  • Use interpretation services effectively and report inefficiencies.
  • Contribute to policy development, clinical governance, audit and continuous improvement processes.
  • Manage and support professional development of team members, including appraisal and delegation.
  • Undertake nurse prescribing when appropriate and in accordance with trust policy.
  • Maintain accurate, up‑to‑date records in line with NMC guidelines and trust policy.
  • Attend MDT and safeguarding meetings as a professional advocate for the trust and patients.
Qualifications & Experience

Essential

  • Registered Nurse (Level
    1)
  • Current UK professional registration
  • Recent nursing and clinical experience leading to autonomous decision‑making
  • Evidence of training/teaching others
  • Understanding of government legislation and documentation related to the specialist client group
  • Knowledge of factors impacting the health of BME populations, gypsies/travellers, and homeless individuals
  • A current driving licence and access to a car during the working day (reasonable adjustments considered)
  • Ability to undertake duties and demands of the post with a satisfactory sickness record over the previous two years
  • Excellent communication and leadership skills; innovative practice; health assessment skills; IT skills
  • Flexible working capability, including out‑of‑hours
  • Presentation skills and reliable attendance record

Desirable

  • Experience working within primary care
  • Previous experience with ethnic minorities, using interpreters, and communicable disease management (e.g., TB)
  • Recorded nurse prescriber
  • Experience in related specialist areas such as sexual health or mental health
  • Group facilitation skills and clinical supervision/appraisal skills
  • Additional nursing practitioner qualifications (e.g., DN, HV)
