Job summary

We are looking for an enthusiastic and committed Practice Nurse to join our friendly GP practice for 10 hours per week, working on Fridays.

This is an excellent opportunity for an experienced Practice Nurse looking for a regular, part-time role within a supportive and well-established practice team.

We recognise that Practice Nurses have different areas of experience and expertise, and we do not expect the successful candidate to have experience in every aspect of the role. We are keen to welcome someone who can bring their own skills and experience to complement our existing nursing provision.

You will have the opportunity to provide a varied range of nursing care, including preventative healthcare, long-term condition management, immunisations and health promotion, according to your training and competencies.

Main duties of the job

The successful candidate will provide a range of planned nursing care to our registered patient population, working alongside our existing Practice Nurse and wider clinical team.

The role will include routine nursing appointments, health promotion, immunisations, clinical observations, wound care and long-term condition reviews, depending on your experience and competencies.

You will support patients with the management of their health conditions, provide appropriate education and self-management advice, undertake relevant monitoring and investigations, and ensure patients receive appropriate follow-up or onward referral when required.

You will work autonomously within your professional scope of practice, whilst having the support of the wider practice team and access to GP colleagues when clinical advice or escalation is required.

You will also contribute to clinical governance, accurate record keeping, safeguarding, infection prevention and quality improvement within the practice.

Responsible To: GP Partners / Practice Manager

Professional Accountability: The post holder will be professionally accountable to the Nursing and Midwifery Council (NMC) and will work within the NMC Code of Professional Standards of Practice and Behaviour.

JOB DESCRIPTION

To provide high-quality, evidence-based nursing care to patients registered with the Practice, working independently and in partnership with the GPs and wider multidisciplinary team.

The post holder will assess, plan, implement and evaluate nursing care and treatment within their professional competence, in accordance with current national guidance, Practice policies, agreed clinical protocols and the patient's informed consent.

The Practice Nurse will contribute to the delivery of preventative healthcare, health promotion, immunisation, screening and long-term condition management, whilst ensuring a high standard of patient safety, confidentiality and professional practice.

The exact range of duties undertaken will be dependent upon the post holder's qualifications, training, experience and demonstrated clinical competencies.

MAIN DUTIES AND RESPONSIBILITIES

The Practice Nurse will undertake appropriate clinical duties, which may include:

  • Assessing patients and providing appropriate nursing care and advice.
  • Maintaining accurate, contemporaneous and comprehensive patient records, both electronic and written where applicable.
  • Undertaking clinical observations including blood pressure, pulse, temperature, respiratory rate, oxygen saturation, height, weight and BMI as clinically appropriate.
  • Venepuncture and the appropriate collection, handling and processing of specimens.
  • Performing ECGs where trained and competent.
  • Undertaking spirometry where appropriately trained and competent.
  • Providing wound care and dressing procedures.
  • Providing health promotion and lifestyle advice.
  • Providing smoking cessation advice and signposting to appropriate services.
