About the Post
This post is for two permanent full‑time social workers in the Adult Social Care & Health service. The role involves working with individuals, families, carers and communities to promote independence and wellbeing, using a strengths‑based and preventative approach. The position requires a recognised social work qualification, registration or pending registration with Social Work England and will report within the local authorities.
Key Responsibilities- Provide assessment, care planning and review in line with the Mental Capacity Act 2005, Care Act 2014, Human Rights 1998 and Making Safeguarding Personal (MSP) legislation.
- Conduct risk assessments, preventative work and develop community‑based solutions using resources such as reablement and telehealth care.
- Construct value‑for‑money care packages and commission/arrange appropriate services.
- Collaborate with agencies and case management teams to deliver coordinated support.
- Maintain clear, accurate records and produce high‑quality assessments.
- Advocate for clients and promote equality, diversity and anti‑oppressive practice.
- Recognised social work qualification.
- Registered with Social Work England or candidate to register during employment.
- Relevant experience with adults, families and communities.
- Knowledge of the Care Act 2014, Mental Capacity Act 2005, MSP and Human Rights 1998 legislation.
- Strong communication skills and ability to work creatively and flexibly.
- Fluency in English.
- Suitable DBS check; overseas applicants may need an overseas criminal record check.
Hybrid working model; 2x37‑hour posts per week with flexible hours. The post is permanent and located within the Place Health & Adult Social Care Directorate.
