Position: Social Worker/ Health Practitioner (2 Posts)

About the Post

This post is for two permanent full‑time social workers in the Adult Social Care & Health service. The role involves working with individuals, families, carers and communities to promote independence and wellbeing, using a strengths‑based and preventative approach. The position requires a recognised social work qualification, registration or pending registration with Social Work England and will report within the local authorities.

Key Responsibilities
  • Provide assessment, care planning and review in line with the Mental Capacity Act 2005, Care Act 2014, Human Rights 1998 and Making Safeguarding Personal (MSP) legislation.
  • Conduct risk assessments, preventative work and develop community‑based solutions using resources such as reablement and telehealth care.
  • Construct value‑for‑money care packages and commission/arrange appropriate services.
  • Collaborate with agencies and case management teams to deliver coordinated support.
  • Maintain clear, accurate records and produce high‑quality assessments.
  • Advocate for clients and promote equality, diversity and anti‑oppressive practice.
Qualifications & Requirements
  • Recognised social work qualification.
  • Registered with Social Work England or candidate to register during employment.
  • Relevant experience with adults, families and communities.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005, MSP and Human Rights 1998 legislation.
  • Strong communication skills and ability to work creatively and flexibly.
  • Fluency in English.
  • Suitable DBS check; overseas applicants may need an overseas criminal record check.
Working Conditions

Hybrid working model; 2x37‑hour posts per week with flexible hours. The post is permanent and located within the Place Health & Adult Social Care Directorate.
