About the role: The Person al Advisor works closely with care‑experienced young people aged 16‑25 to support their transition to adulthood. The role involves developing and implementing Pathway Plans with measurable goals, acting as an advocate for the young people, and liaising with partner agencies to secure the best outcomes. You will provide individualised support, conduct risk assessments, maintain accurate records, and contribute to team meetings and service improvement.

• Work with care‑experienced young people aged 16‑25 to support their transition to adulthood
• Develop and implement Pathway Plans with measurable goals
• Advocate for young people, liaise with partner agencies and services
• Provide individualised support to help young people achieve personal aspirations
• Carry out risk assessments and maintain accurate records
• Attend team meetings and contribute to service improvement

Qualifications and experience:

• Good communication skills in English
• Previous experience working with young people in care preferred
• Ability to work independently and as part of a multidisciplinary team
• Strong organisational and analytical skills
• Mandatory Enhanced DBS check

Legal and compliance: Must hold the necessary visa or immigration status. The Council does not sponsor immigration. Overseas criminal record checks are required if previously lived overseas.
