We are seeking an enthusiastic and forward‑thinking General Practitioner with a keen interest in acute medicine and frailty to join our acute medicine department as a peer professional. This position is ideally suited for a GP who wishes to broaden and enhance their clinical expertise by working within a multidisciplinary team that is committed to delivering high‑quality patient care in the community.

The successful candidate will collaborate with the Single Point of Access (SPoA) hub and the Acute Hospital at Home (AHAH) service to provide Hospital Outreach. This innovative service, comprising Advanced Clinical Practitioners, Band 7 nurses, and doctors, focuses on delivering rapid assessment and intervention in patients’ homes. The aim is to provide prompt clinical management to patients experiencing a crisis, allowing them to receive necessary treatments such as oxygen therapy, intravenous medications, and wearable monitoring outside of a traditional hospital setting.

This approach is particularly beneficial for individuals living with frailty or complex dependencies who may face significant barriers to hospital attendance.
