Job Summary

We are looking for an enthusiastic experienced Clinical Pharmacist to join our Clinical Pharmacy Team in the Barnstaple Alliance Primary Care Network.

The role will focus on managing patients with long‑term conditions and working closely with GPs, other practice staff and the primary healthcare team to support safe, cost‑effective, evidence‑based prescribing.

Hours are flexible between 25 and 37.5 hours a week to suit the right candidate.

Location:

Brannam Medical Centre, Barnstaple, with travel to Fremington, Litchdon and Queens Medical Centres.

Salary: £46,000–£59,000 WTE, depending on qualifications and experience.

Main duties of the job
  • Work as part of a multi‑disciplinary team in a patient‑facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  • Collaborate with clinicians, the Clinical Pharmacy team, MDTs, the wider PCN and member practices to identify and clinically manage a caseload of patients with long‑term conditions.
  • Evaluate clinical information from examination and history taking; provide specialist expertise in medicines and address public‑health and social‑care needs while tackling inequalities.
  • Provide leadership on person‑centred medicines optimisation and quality improvement, contributing to the quality and outcomes framework and enhanced services.
  • Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social‑care system.
  • Be part of a professional clinical network and have access to appropriate clinical supervision.

Job Description

Core responsibilities for clinical pharmacists in delivering the additional PCN health services to patients are:

  • Work as part of a multi‑disciplinary team in a patient‑facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  • Complete training to become a prescriber and work alongside the general practice team to manage chronic diseases and perform clinical medication reviews, particularly for the elderly, people in care homes, and those with multiple comorbidities (frailty, COPD, asthma) and learning disabilities or autism.
  • Identify and clinically manage a caseload of patients with long‑term conditions in partnership with clinicians, the Clinical Pharmacy team, MDTs, the wider PCN and member practices.
  • Provide a holistic approach to patient care, including lifestyle advice and opportunities for patient peer group sessions focused on long‑term conditions and lifestyle management.
  • Evaluate clinical information from examination and history taking; instigate necessary investigations and interpret findings, liaising with the Clinical Pharmacy team and PCN practice staff as appropriate.
  • Provide specialist expertise in medicine use, addressing public‑health and social‑care needs and tackling inequalities.
  • Lead on person‑centred medicines optimisation, ensuring prescribers conserve antibiotics in line with local antimicrobial stewardship guidance, and drive quality improvement. Support patients to take their medications effectively, reduce waste and promote self‑care.
  • Support further integration of general practice with the wider healthcare teams (community and hospital pharmacy) to improve patient outcomes, access to care and practice workload.
  • Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health‑and‑social‑care system.
