Mental Health and Wellbeing Practitioner

Join our friendly and dynamic Community Mental Health Team as a Band 5 Mental Health Practitioner, and play a vital role in supporting adults on their recovery journey
.

Taw CMHT is based in Barnstaple, covering surrounding areas.

Main duties of the job
  • Assessments and caseload management, care planning, including risk and future support plan.
  • Delivery of interventions, psychosocial interventions such as brief structured work.
  • Promote recovery, independence and social inclusion.
  • Risk management and safeguarding
  • Clear and accurate recording within a patient's clinical notes.
  • Collaborative working with the MDT, liaising with GP's and other agencies from voluntary or statutory sectors.

responsibilities

To provide high-quality, person-centred mental health care within a multidisciplinary Community Mental Health Team, contributing to the assessment, planning, implementation, and evaluation of care for adults with mental health needs, in line with NICE guidance, and Trust policies.

The post holder will work under appropriate supervision, managing a defined caseload and supporting recovery-focused interventions.

Person Specification Experience
  • Experience of working within a mental health setting.
  • Experience of dealing with vulnerable patients.
  • Experience of managing a caseload of clients with a range of mental health difficulties.
  • Able to demonstrate a sound theoretical knowledge of mental health practice.
  • Previous experience of working within a community setting either voluntary or statutory sector.
  • Experience of working in a Multi-disciplinary and Multi-agency environment.
Qualifications
  • An appropriate up to date professional mental health qualification eg RMN, OT or social worker on the appropriate professional body register and hold current registration with no restrictions.
  • Training to level 2 in Safeguarding children and young people: roles and competencies for Health Care staff.
  • Post registration training in relevant field.
