An exciting opportunity has become available to become part of the Hospital Discharge Team based at North Devon District Hospital. The team is made up of Social Workers, Social Care Assessors, Nurses and Paramedics with the aim to support people to access the most appropriate pathway to facilitate their discharge from hospital.

We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers and other professionals. You will have experience of working in a health or social care environment and have a sound understanding of human needs. You will also ideally have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team.

You should be an effective communicator, both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have effective time management skills and be able to work under pressure.

You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to effective customer service and to achieving positive outcomes for vulnerable people.

In return, you will be part of a friendly, supportive and experienced team with regular supervision, appraisal, regular team meetings and access to learning and development opportunities.
