PCN Digital and Transformation Project Manager

The Barnstaple Alliance Primary Care Network (PCN) is recruiting a new and innovative Digital and Transformation Project Manager who will work closely with the PCN Management Team to support the transformation and sustainability of general practice and the care services the PCN provides by developing digital solutions and capacity whilst improving accessibility.

Hours: 18–25 hours per week, to be agreed with the successful applicant.

Salary: £31,000 – £37,037 FTE, depending on experience and qualifications.

Main Duties

The Digital & Transformation Project Manager role aims to support transformation and digital initiatives of the Barnstaple Alliance PCN to continue to support good access for patient care and the overall sustainability of general practice and PCN-wide services.

Ensure digital transformation supports all patients, including those who may face barriers to accessing digital services, helping to reduce inequalities and improve access to care. The role will also identify and tackle health inequalities, improving the quality of the patient population. Population Health Management will be used to understand future demand, those most vulnerable, and to proactively seek solutions.

Responsibilities
  • Work with the Management Team to create a vision for service improvements, identify opportunities for tangible transformation, and provide leadership to deliver changes collaboratively.
  • Help develop strategy related to transformation and improvement that makes a difference to patients and staff.
  • Provide practical project management support to practices in implementing and sustaining agreed changes.
  • Support delivery of transformation activity in general practice to improve patient access while protecting workforce quality and continuity of care.
  • Support practices and staff to optimise the use of existing digital functionality and new technology.
  • Keep the Management Team and member practices updated on the progress of current initiatives.
  • Assess and review the PCN’s transformation maturity, capacity, and local support needs.
  • Develop relationships with peers across other PCNs to share learning and expertise.
  • Work with the PCN Operational Manager to manage compliance with Data Protection Regulations/GDPR, including DPIAs, Data Sharing Agreements, and assessments.
  • Lead or participate in relevant working groups, project groups, service meetings, and other forums to agree changes and resolve issues.
  • Drive integrated working, reduce duplication, and improve efficiency between practices.
  • Disseminate new information across member practices and coordinate shared learning events.
  • Work with local community and voluntary organizations to support patients who struggle to access services through digital routes and develop practical solutions.
  • Work with the Integrated Care System (ICS) to request specialist IT and estates expertise required to deliver transformation plans.
Data, Population Health Management and Health Inequalities
  • Identify digital solutions to support the development, resilience, and future-proofing of the PCN.
  • Improve adoption of new technology to enhance patient access and experience.
  • Review and improve digital maturity.
  • Take a data‑driven approach, using evidence to understand challenges and track impacts of changes.
  • Effectively present and communicate data to practices and staff.
  • Interpret data and highlight variation for discussion.
  • Help staff understand limitations of data sets and supplement with other information.
  • Use data to tackle health inequalities and improve outcomes, focusing on PCN needs and wider NHS strategies.
  • Understand demand, capacity and activity to drive improvement.
  • Support population health management, tackle inequalities, and improve health outcomes.
  • Identify PCN pressure points and proactively develop services for sustainability.
  • Support adoption of national and local initiatives, including integrated working at neighbourhood and place level.
