Berlin Health & Rehabilitation Center is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Barre, Vermont.
Pay: $42–$52 per hour
We provide person-centered care focused on helping residents and patients improve function, maintain independence, and achieve meaningful rehabilitation goals. Our interdisciplinary team works collaboratively to provide high-quality, compassionate care.

Physical Therapist Responsibilities- Evaluate patients and residents to determine their physical therapy needs, functional limitations, and rehabilitation goals.
- Develop individualized treatment plans based on evaluation findings and patient goals.
- Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, function, and independence.
- Administer medically prescribed treatments in accordance with the established plan of care.
- Monitor patient progress and response to treatment and modify interventions as clinically appropriate.
- Provide education and guidance to patients, families, and caregivers regarding treatment plans, exercises, mobility, and safety.
- Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team to coordinate patient care.
- Document evaluations, treatment sessions, progress, and other required clinical information accurately and timely.
- Maintain compliance with applicable federal and state regulations, professional standards, and facility policies.
- Participate in required meetings and other duties as assigned by the Director of Rehabilitation.
- Active and valid Physical Therapy license in Vermont.
- Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.
- Current knowledge of physical therapy treatment practices, techniques, and interventions.
- Familiarity with Net Health or similar electronic documentation systems preferred.
- Strong clinical, communication, organizational, and interpersonal skills.
- Ability to work effectively with patients and residents with a variety of needs and abilities.
- Ability to collaborate effectively with an interdisciplinary healthcare team.
- Commitment to providing compassionate, patient-centered care.
- Must be in good standing with all applicable licensing boards and regulatory agencies.
