Job Title: Home Health RN Case Manager

Schedule: 40 hours/week

Shift: 4x10 days (10 hrs) Thursday - Monday

Pay Range for Traveler: $2,800.00- $2,847.00 Weekly DOE

Job Summary

We are looking for the Home Health RN Case Manager who is responsible for coordinating, delivering, and managing high-quality nursing care to patients in their homes. This role ensures patient needs are assessed, care plans are developed, and services are delivered in accordance with clinical standards, regulatory requirements, and agency policies.

• Conduct comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
• Develop, implement, and oversee individualized patient care plans.
• Provide skilled nursing care such as wound care, medication administration, IV therapy, and disease management education.
• Monitor patient condition changes and adjust care plans accordingly.
• Complete and submit accurate, timely OASIS documentation and visit notes.
• Coordinate multidisciplinary services including physical therapy, occupational therapy, social work, and home health aides.
• Communicate regularly with physicians regarding patient status and care plan updates.
• Act as the primary case manager, ensuring continuity of care and appropriate visit frequency.
• Educate patients and family members on disease processes, medications, and self-care strategies.
• Participate in case conferences, team meetings, and quality improvement initiatives.
• Ensure compliance with state, federal, and agency regulations and standards.
• Manage a caseload efficiently to meet productivity expectations.
• Provide compassionate, patient-centered care in a home setting.

• Experience:
  
  Minimum 1 year of home health and case management experience required.
• OASIS experience required.
• Certification: BLS
