Position Purpose
:
Provides certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures and at the direction of supervisor(s).
- Licensed Nursing Assistant in good standing with the state.
- High school diploma or equivalent.
- Current CPR/BLS certification.
- Must have no disciplinary action in effect against certification.
- At least one year of experience as a certified nursing assistant in long‑term care.
- Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
- Assists residents with activities of daily living, lifts, turns, transports, and positioning.
- Coordinates dining room services at assigned mealtimes, including set‑up, clean‑up, meal tray delivery, feeding assistance, and documentation of meal intake.
- Delivers nutritional supplements at assigned times and ensures intake; documents intake accordingly.
- Weighs residents per facility policy; records weight and reports changes to nurse and supervisor.
- Tracks condition of the resident’s skin; reports any pressure areas, skin breakdown or skin tears to nurse and supervisor.
- Assists nursing staff in carrying out toileting program activities.
- Completes flow sheets daily to indicate that specified tasks were done; documents the progress of each resident as directed by supervisor.
- Keeps nurse and supervisor informed of factors that interfere with ability to perform assigned work (e.g., resident refusal, reassignment).
- Reports changes in a resident’s condition to the resident’s nurse and supervisor.
