About The Aspen PCN
The Aspen Primary Care Network first became operational in 2010, serving three communities. Since then, we have grown to include partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso, High Prairie and Wabasca, Alberta. In total, the PCN has 43 participating physicians serving approximately 30,000 residents within our region.
Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions.

Position Summary
The Aspen Primary Care Network is currently seeking a 1.0 FTE Permanent Licensed Practice Nurse (LPN)/Family Practice Nurse (FPN). The LPN/FPN works collaboratively with the multidisciplinary team, physicians, and community agencies to enhance the delivery of Primary Care Services of all age groups. The LPN/FPN works within their scope of prac tice to provide high-quality, evidence-based care to patients with a variety of medical conditions.
What You Will Do
- Works to full scope of practice with an emphasis on providing comprehensive care to patients of all ages
- Provides full health services based on highly developed nursing knowledge and skills
- Conducts assessments to identify needs and collaborates with patients to set priorities and plans for care
- Provides short term interventions and patient education
- Provides assistance with navigation of the health system
What You Need To Succeed
- Must be a resident of Alberta
- Current CLPNA registration (Non-Provisional License)
- Current BLS
- Five years or more related nursing experience preferred
- Experience working in a Primary Care Medical Clinic is preferred
- Experience working with Microsoft office suite and EMR systems an asset
- Successful completion of Medication Administration and Injection courses
- Advanced skills in health promotion, health screening, assessment and education
- Strong knowledge and understanding of Indigenous culture and communities considered an asset
- Ability to practice in a culturally safe manner
- Current RCMP Criminal/Vulnerable Sector Check (within 30 days)
- Completion of reference checks
