Clinical Pharmacist
Reporting to the Manager, Clinical Pharmacy Services, the Clinical Pharmacist is an integral member of the Pharmacy Team. The Clinical Pharmacist is accountable and responsible for completing patient medication assessments, ensuring optimal medication therapy and minimizing potential for adverse events. They are also accountable for providing accurate medication information to the interdisciplinary team. The pharmacist promotes safe and appropriate use of medications in a multidisciplinary setting to minimize risk and optimize health care resources as they pertain to medications.
Responsibilities:
Complete patient medication assessmentsEnsure optimal medication therapy and minimization of potential adverse events.Review of lab information to ensure appropriate drug therapy.Participate in patient rounds in clinical area of practice.Participate in drug distribution, including accurate order entry of physician orders.Provide pharmacists and other healthcare professionals with acquiring relevant drug information.Actively reviews and provides recommendations on various document submissions relating to clinical area of coverage.Work collaboratively with interdisciplinary team on patient care unitsComplete appropriate medication safety audits, such as high alert medication, VTE, that align with Accreditation Canada standards.Participate in the education and mentoring of pharmacy students and new staff.Participate in the education and ongoing learning of nursing staff.Other duties, as assigned.Exhibits the core values of Royal Victoria Regional Health Centre:

Registered Pharmacist, in good standing with the Ontario College of Pharmacists (Part
A)Bachelor of Science or Pharm
D degree in PharmacyExperience:
Post-graduate Pharmacy Residency Program (ACPR), an assetMinimum two (2) years of experience in a hospital pharmacy setting, preferredCompetencies:
