In this 12-month contract role, take charge of creating Individual Rehabilitation Plans based on client-centered goals. You will collaborate closely with team supervisors and Behaviour Consultants, maintaining communication with clients and their families to report progress. You will also oversee the scheduling of rehabilitation workers as required.
Key Responsibilities:
• Implement and track Individual Rehabilitation Plans
• Liaise with social services and health care providers
• Conduct RAI CHA assessments and necessary follow-ups
• Report on client progress and incident management
• Assist with service delivery in community settings
Requirements:
• Degree preferred in Human and Social Services
• At least three years of relevant Human Services experience
• Experience in report writing and group facilitation
• Valid CPR/First Aid certification preferred
• Strong interpersonal skills and team leadership experience
