Key Responsibilities:

• Conduct in-home assessments and evaluations

• Create evidence-based treatment plans tailored to clients

• Implement therapeutic exercises and rehabilitation strategies

• Educate clients and families on mobility aids and home safety

• Collaborate with interdisciplinary teams for seamless care

Requirements:

• Registered with the College of Physiotherapists of Ontario

• Experience in home and community care preferred

• Proficiency in mobility and falls assessments

• Ability to work independently in the community

• Valid driver's license and reliable transportation
