As a Health Care Connector, your mission is to facilitate connections between patients and primary care providers within the health care system. You will play a crucial role in maintaining relationships with patients while supporting their attachment to health resources. This dynamic role requires strong assessment skills and the ability to communicate effectively with a diverse patient population.
Key Responsibilities:
• Help patients navigate the health care landscape
• Keep in touch with patients on the priority list
• Provide assessments and referrals as needed
• Collaborate with the inter-professional team
• Document interactions following policy standards
Requirements:
• Valid Registered Practical Nurse certification
• 3–5 years of relevant nursing experience
• Strong skills in communication and teamwork
• Experience with vulnerable populations preferred
• Competent in using technology and software programs
