Position: Registered Practical Nurse - Way Home (H2)

As a member of an interdisciplinary team, the Registered Practical Nurse (RPN) provides patient-centered nursing care within the Way Home Unit, supporting patients transitioning from hospital to home. The RPN may be redeployed to other patient care areas as organizational needs arise and may also provide care in the community through the OSMH at Home program when assigned. Working within the scope of practice and in accordance with College of Nurses of Ontario standards, Hospital policies, and Patient Care Services guidelines, the RPN delivers safe, compassionate, and evidence-informed care while contributing to continuity of care, patient safety, and collaborative practice.
Key Responsibilities
Meets the College of Nurses of Ontario Standards of Nursing Practice for Registered Practical Nurses and practices in accordance with Hospital and Patient Care Services policies and procedures.Provides direct patient care primarily within the Way Home Unit, supporting patients transitioning from hospital to home, and may be redeployed to other patient care areas as operational needs arise.Provides nursing care in the community through the OSMH at Home program when assigned, including home visits and delivery of care in accordance with established care plans.Organizes and implements nursing care plans to meet the assessed needs of each assigned patient.Performs nursing interventions including head-to-toe assessments, vital signs, applying aseptic techniques and sterile dressing changes, wound, ostomy, catheter, and drain care, specimen collection and monitoring nutritional intake.Administers medication and monitors therapeutic effects and documents patient responses accurately and in a timely mannerProvides pre-operative and post-operative personal and comfort care as required.Monitors and support established respiratory and intravenous therapies within the scope of practice.Utilizes critical thinking to assess changes in patient condition and initiates consultation or transfer of care as appropriateSupports safe transitions of care through patient and family education, reinforcement of discharge plans and referrals to appropriate community resources.Provides cognitive, social, emotional, and spiritual supports to patients and familiesUtilizes human and material resources effectively and responsibly.Participates in the orientation and mentorship of new team members and nursing students, including preceptorship activities as assigned.Maintains professional competence through ongoing learning, attendance at education sessions and in-services, and self-assessment of professional development needs.Assists in maintaining the established standards of care through individual performance, participation in quality improvement initiatives, audits, and hospital committees as required.Maintains awareness of current developments in nursing practice.Responsible for the safety and welfare of patients.Functions in accordance with established hospital emergency procedures.Promotes effective communication and collaboration within and across departments.Participates as a member of the interdisciplinary team, promoting effective liaisons between patients, family and physicians, healthcare team members, and community agencies. Encouraging referrals to appropriate community resources.Maintains awareness of current developments in nursing practice and relevant legislation, including the Regulated Health Professions Act (RHPA).Participates in assessing health needs and promoting health education and maintaining through patient and family teaching programs.May be utilized in other patient care areas during a shift based on patient care needs and operational requirements.Performs other related duties as may be assigned by the Program Manager or designateQualifications
Education
Registered and in good standing with the College of Nurses (CNO) as a Registered Practical Nurse.Current Basic Cardiac Life Support (BCLS) is required; CPR Level HCP (Health Care Provider) will be accepted as an equivalent.IV certification or the ability to obtain certification within six (6) months of hire. Experience with IV initiation may be considered.Experience
A minimum of two (2) years of recent experience in an Acute Care or Long-Term Care setting is preferred.A minimum of one (1) year of recent medical/surgical experience is preferred.A minimum one (1) year of experience in Cardiac, and Renal Care is preferred.Skills and Traits
Must be able to meet the physical and emotional responsibilities of the role, including standing and walking for extended periods, frequent and repetitive pushing, pulling, bending, and lifting up to 50 pounds, as well as safely operating equipment.Ability to meet the physical demands of the position, as outlined in the Physical Demands Analysis (PDA).Demonstrates proficiency in written and verbal communication, with strong interpersonal and organizational skills.Povides excellent customer service and contributes positively…
