As a member of an interdisciplinary team, the Registered Nurse (RN) on the Acute Medicine Unit (Soldiers’
1) provides patient-centered nursing care to support the assessment, planning, implementation, and evaluation of care for assigned patients. Practicing in accordance with College of Nurses of Ontario standards, Hospital policies, and Patient Care Services guidelines, the RN delivers safe, compassionate, and evidence-informed care within the acute medicine environment. The RN demonstrates clinical leadership, critical thinking, and accountability while contributing to patient safety, continuity of care, quality improvement, and collaborative practice.
Key Responsiblities
Meets the College of Nurses of Ontario Standards of Nursing Practice for Registered Nurses and practices in accordance with Hospital and Patient Care Services policies and procedures.Assesses patient needs and develops, implements, and evaluates individualized plans of nursing care each assigned patient.Applies evidence-based practice and clinical judgment to coordinate patient care activities and identify appropriate resources and supports.Conducts comprehensive nursing assessments and utilizes critical thinking to assess changes in patient status (complexity, stability), initiating consultation, escalation, or transfer of care as required.Provides direct nursing care including medication administration, wound management, pre- and post-operative care, patient monitoring, and coordination of care.Collaborates with the interprofessional team to coordinate care, support safe transitions, and promote positive patient outcomes.Monitors patient care, workflow processes and outcomes and participates in quality improvement initiatives to optimize care delivery.Utilizes human and material resources effectively and responsibly.Assumes responsibility for patient safety and contributes to a culture of safety.Functioning in accordance with established hospital emergency procedures.Provides leadership and support to team members and performs the Charge nurse role when assigned.Participates in the orientation, mentorship and support of new team members and students to the unitCollaborates with external education partners to support student learning experiences.Maintain professional competence through ongoing education, participation in in-services, and self-assessment of professional development needs.Participates as a member of the interprofessional team, promoting effective communication and collaboration with patients, families, physicians, healthcare team members and community agencies. Encouraging referrals to appropriate community resources.Supports patient and family education, health promotion, and referrals to appropriate community resources.Assists in maintaining established standards of care through individual performance, participation in hospital committees, and engagement in quality improvement and patient flow initiatives.Maintains awareness of current developments in nursing practice and relevant legislation, including the Regulated Health Professions Act (RHPA).Promotes effective communication and collaboration within and across departments.May be utilized in other patient care areas during a shift based on patient care needs and operational requirements.Performs other duties as may be assigned by the Program Manager and/or designate.
Qualifications
Education
Registered and in good standing with the College of Nurses (CNO) as a Registered Nurse.Completion of an approved nursing program BScN or equivalent is required.Current Basic Cardiac Life Support (BCLS) is required; CPR Level HCP (Health Care Provider) will be accepted as an equivalent.Completion of Coronary Care I certificate or willing to obtain within 6 months of hire, is required.IV certification or the ability to obtain certification within six (6) months of hire. Experience with IV initiation may be considered.Experience
Minimum two (2) years medical/surgical experience is required.Minimum one (1) year of experience in Cardiac Care is required.Previous work experience in Telemetry and Renal Care is preferred.Skills and Traits
