As a Registered Practical Nurse, you will play a vital role in supporting patient transitions across departments, including inpatient units and the Emergency Department. Your responsibilities will encompass assessment, treatment, and documentation while aligning with the standards mandated by the College of Nurses of Ontario. This position is designed for those with a passion for enhancing care delivery.
Key Responsibilities:
• Assist in the care of admitted and discharge-pending patients
• Perform thorough admission assessments and document patient details
• Communicate timely transfer of accountability (TOA)
• Become fluent in P4R metrics and collect simple statistics
• Provide feedback for care improvement
Requirements:
• Current RPN Registration with the College of Nurses Ontario
• Minimum of one year recent experience in Acute Nursing Care
