Key Responsibilities:
• Conduct detailed assessments and create effective care plans
• Lead client care as the designated primary provider

Requirements:
• Active Certificate of Competence from the College of Nurses of Ontario
• Must be available to work flexible shifts, including weekends
• Required clear criminal background and updated immunization
• Willingness to achieve key certifications from SE Health
• Current CPR certificate is necessary
