As part of our dedicated Transitions in Care Team, you’ll play a crucial role in enhancing care delivery. This temporary position allows you to support patients through their transitions, collaborate across departments, and contribute towards key performance goals. If you're focused on seamless patient care and have a passion for nursing, this role is perfect for you.
Key Responsibilities:
• Assess, treat, and provide rehabilitative care to patients
• Assist in care for admitted and discharge-pending patients
• Perform admission assessments and document interventions
• Communicate systematic transfer of accountability
• Collect statistics and provide feedback for improvement
Requirements:
• Current RPN Registration with the College of Nurses Ontario
• Minimum one year of Acute Nursing Care experience
