SE Health invites you to join their mission in a full-time permanent role that emphasizes autonomy and teamwork in delivering exceptional care. With a minimum of two years’ nursing experience, you'll engage in comprehensive support activities while developing individualized care plans tailored to each client. Participate actively as a primary care provider within an interdisciplinary team, enhancing the overall care experience.
Key Responsibilities:
• Provide holistic and comprehensive client care plans
• Act as primary contact for interdisciplinary team coordination
• Lead assessment and facilitate transitions of care
• Organize caregiver meetings for supportive practices
• Foster self-care and health promotion strategies for clients
Requirements:
• Registration in good standing with Ontario's nursing college
• At least two years of relevant nursing experience
• Clinical leadership and case management experience
• Strong interpersonal and organizational abilities
• Capable of traveling with a valid driver’s license
