Advance your career with the Barrie & Community Family Health Team as a Nurse Practitioner. This permanent role focuses on comprehensive patient care, including health promotion and chronic disease management.
The BCFHT is seeking a dedicated Nurse Practitioner for a 0.2 FTE position. You’ll collaborate with a team of healthcare professionals to improve patient health and quality of life. Your expertise will aid both acute episodic care and chronic disease management, fostering independence through comprehensive assessments and patient services.
Key Responsibilities:
• Provide primary health care according to established standards
• Conduct health promotion and disease prevention initiatives
• Enhance community access to assessments and treatments
• Collaborate with interdisciplinary team members for patient care
• Coordinate chronic disease management and palliative care
Requirements:
• Current registration with the College of Nurses of Ontario
• Completion of a recognized NP program: NP-PHC
• Experience with electronic medical records
• Strong communication and problem-solving skills
• Ability to travel within the community
