Community-Focused Registered Practical Nurse

In this full-time position at Para Med, you will conduct scheduled visits to assist patients throughout Muskoka. This role welcomes new graduates and those seeking work-life balance while making a significant impact on patient comfort and independence 'll collaborate with Registered Nurses for complex care needs and develop long-term therapeutic relationships with patients.

Key Responsibilities:

• Provide ethical and safe nursing care during community visits

• Assess individual patient needs using appropriate tools

• Execute, monitor, and adjust care plans as required

• Build rapport and trust with patients collaboratively

• Maintain and update professional patient records diligently

Requirements:

• CNO registration as a Registered Practical Nurse in good standing

• Valid driver's license and access to a reliable vehicle

• Experience in community care is preferred

• Flexibility and reliability in travel to patient residences

• Willingness to provide on-call coverage as needed
