Key Responsibilities:
• Participate in community visits addressing patient needs
• Deliver ethical and patient-focused nursing care
• Assess and document individual patient conditions and needs
• Continuously monitor and tailor care plans as necessary
• Enhance patient engagement through rapport and trust
Requirements:
• Registered as a practical nurse with the CNO
• Possession of a valid driver's license and vehicle access
• Experience in community care preferred but not required
• Reliability and willingness to travel within a designated area
• Must be available for on-call coverage, including after hours
