In this Part-Time Nurse role, you will be integral to providing excellent client care in the community. Your responsibilities will include conducting assessments, developing tailored care plans, and working collaboratively with care teams. Key Responsibilities:
• Complete assessments and implement personalized care plans
• Serve as the primary provider for a client caseload
• Assist clients in transitioning to community support services
Requirements:
• Must be registered with the College of Nurses of Ontario
• Hold a current CPR certificate and have a valid driver's license
• Passion for community work and excellence in nursing practice
• Background check, immunization records, and physical dexterity required
• Availability for flexible schedules including days and weekends
