Key Responsibilities:
• Conduct thorough assessments to determine patients' needs
• Implement and continuously evaluate care plans for patients
• Engage with patients to create trusting relationships
• Maintain accurate records of patient interactions
• Fulfill on-call responsibilities for patient management
Requirements:
• Valid RN registration with the CNO
• Holding a valid driver’s license with vehicle access
• Previous community care experience is a plus
