As a Registered Nurse, you will conduct detailed assessments, design care plans, and guide clients through their healthcare journeys. This position demands a valid Nursing registration, a current CPR certificate, and a love for community care. You will actively collaborate with other healthcare professionals to optimize client transitions to external support services.
Key Responsibilities:
• Assess clients and develop individualized care plans
• Lead care for a designated caseload as primary provider

Requirements:
• Registered with the College of Nurses of Ontario
• CPR certification and valid driver's license required
• Ability to perform physically demanding tasks
