Key Responsibilities:

• Coordinate follow-up appointments and referrals for patients

• Manage clinical scheduling and patient intake processes

• Support health promotion activities and community care initiatives

Requirements:

• Current Registered Practical Nurse registration with CNO

• 1-2 years' experience in primary care or community settings

• Proficiency in Electronic Medical Records systems

• Excellent communication and active listening skills

• Understanding of Indigenous community health needs
