As a Nurse Practitioner (MRP) at BCFHT, you’ll provide primary health care across various conditions and collaborate with interdisciplinary teams.
Your role will focus on patient education, chronic disease management, and enhancing community access to healthcare. You will be responsible for managing an assigned panel of patients, ensuring high standards of care are maintained.
Key Responsibilities:
• Deliver comprehensive primary care according to governing standards
• Focus on health promotion and disease prevention activities
• Manage chronic diseases and provide palliative care
• Coordinate with an interdisciplinary team for patient care
Requirements:
• Current registration with the College of Nurses of Ontario
• Completion of NP-PHC program or equivalent qualification
• Experience with inter-professional teams
• Proficiency in electronic medical records
• Strong communication and problem-solving skills
