Nurse Practitioner at BCFHT

Elevate community health as a Nurse Practitioner (MRP) with the Barrie and Community Family Health Team in Barrie, Ontario. Your focus will be on patient-centered care and health promotion initiatives.

In this role, you’ll work collaboratively within a multi-disciplinary team to deliver primary health care to patients of all ages. As an NP, you’ll emphasize chronic disease management, support self-management, and facilitate access to diverse health services. You’ll have the opportunity to implement treatment strategies and provide acute care where needed, contributing to the overall health of the community.

Key Responsibilities:

• Provide patient-centered primary care aligned with strategic goals

• Implement disease prevention and health promotion strategies

• Enhance access to essential health services for the community

• Consult with healthcare providers to coordinate care plans

Requirements:

• Registration with the College of Nurses of Ontario

• Completed Primary Care Nurse Practitioner program

• Ability to work extended hours

• Experience in electronic medical records

• Demonstrated teamwork and communication skills
