Care Partners seeks a Personal Support Manager for its Leadership Team in Barrie and area. The position provides coordination, supervision, and coaching while developing effective care plans. It also requires knowledge of case management, nursing processes, and Community Health Services best practices.
Key Responsibilities:
Coordinate and supervise Community Support Workers in the field
Develop and maintain effective Community Health Services care plans
Monitor employee performance and apply relevant policies
Promote Community Health Services and maintain public relations
Guide workplace Health and Safety activities
Requirements:
Registered Nurse or Registered Practical Nurse degree or diploma
Three to five years of related experience or equivalent
Registration with the College of Nurses of Ontario
Current CPR Certification and required background checks
Vehicle, regular travel, and excellent English communication skills
Bring nursing processes, case management, and leadership skills to coordinate community care with Care Partners in Barrie and area.
