Hospital Discharge Planning Nurse

Hospital discharge planning nurses coordinate safe transitions, patient flow, and community referrals across MAHC’s Huntsville and Bracebridge sites. This on-site role includes varied shifts.

As a Discharge Planner, you use nursing judgment to coordinate designated patients from admission through discharge. You complete screening assessments, identify barriers, and collaborate with interprofessional teams, families, physicians, and external partners. The work supports timely discharges, admission avoidance, and continuity of care.

• Lead discharge planning from admission through patient discharge
• Complete screening assessments within 24 to 48 hours
• Arrange referrals, transportation, services, and follow-up care
• Support patients at risk of Alternate Level of Care designation
• Complete documentation, medication reconciliation, and provider communication

• Current College of Nurses of Ontario registration in good standing
• Three years of acute care hospital nursing experience
• Current Basic Life Support certification
• Microsoft Office and electronic health records experience
• Availability for days, evenings, nights, and weekends
