As a Registered Nurse at SE Health, you will provide comprehensive assessments and develop personalized care plans tailored to client needs. This role includes collaborating with other healthcare professionals, supporting client transitions, and managing a designated caseload. Your work will directly impact community health and innovate care practices across Canada.
Key Responsibilities:
• Complete detailed client assessments and care evaluations
• Develop and implement tailored care plans based on best practices
• Serve as point of care lead for client caseload management
• Promote client transitions by connecting them with community services
• Collaborate with care team members to ensure comprehensive care
Requirements:
• Current registration with the College of Nurses of Ontario
• Enthusiasm for community care and evidence-informed practice
• CPR certificate and valid driver’s license required
• Experience with lifting and physical demands of the job
• Willingness to complete Saint Elizabeth certifications
