First Case Manager

Position: Full-Time Housing First Case Manager
Enhance lives as a full-time Housing First Case Manager with Empower Simcoe.

Your role focuses on providing support to individuals facing chronic homelessness within the County of Simcoe.

This contract position allows you to work up to 37.5 hours weekly until April 30, 2027. You will utilize your social services education and at least two years of experience in case management, demonstrating your knowledge of Housing First principles and psychosocial rehabilitation. The role requires strong interpersonal skills and the ability to aid individuals in independent living and community integration.

Key Responsibilities:

• Provide community-based case management services

• Conduct comprehensive assessments of individual needs

• Develop and revise support plans based on progress

• Facilitate community involvement and recreational opportunities

• Engage in crisis management and risk assessment consultations

Requirements:

• Community College Diploma or University Degree in social services

• At least 2 years of relevant case management experience

• Knowledge of Housing First and harm reduction principles

• Strong problem-solving, judgment, and communication skills

• Ability to work independently and within a team

Support individuals in achieving independence and stability through professional case management as part of the Housing First initiative.
