Join LOFT Community Services as a Psychogeriatric Case Manager, providing essential support to seniors in Bradford, ON. This onsite role emphasizes community linkages and enhancing clients' quality of life through tailored interventions.

This new Case Manager position involves working closely with older adults experiencing mental health issues and addictions. Key responsibilities include creating and executing care plans, conducting assessments, and maintaining relationships with multidisciplinary care teams. By applying psychosocial rehabilitation principles, you will support clients in achieving their independence and dignity.

Key Responsibilities:

• Create personalized care plans to enhance client independence

• Act as an advocate among care providers and family

• Conduct assessments and manage case documentation

• Provide crisis intervention and support

• Liaise with community service partners for client needs

Requirements:

• Degree in social/human services or equivalent

• Minimum 3 years supporting vulnerable older adults

• Knowledge of community resources and interventions

• Excellent communication and assessment skills
