Position: Veterans Housing Case Manager (Bilingual)

Join CONTACT Community Services as a Bilingual Case Manager for the Housing First Veterans Program. In this role, you'll help Veterans find stability through effective case management in South Simcoe and Georgian Bay.
As a full-time Bilingual Case Manager, your role will focus on supporting Veterans experiencing homelessness. You'll apply Housing First principles, tailoring individualized care plans to meet unique needs and promote long-term well-being. Collaborating with a network of community partners, you will guide Veterans toward achieving independent living and accessing vital services within their communities.

Key Responsibilities:

• Provide community-based case management for Veterans

• Attend appointments and offer accompaniment as needed

• Facilitate engagement in community activities and support

• Assess individual needs comprehensively

• Coordinate with the Housing First team for effective strategies
Requirements:

• Bilingual in English and French

• Educational background in social services or similar fields

• At least 2 years of experience in social services

• Understanding of Housing First and harm reduction

• Valid G license and access to a reliable vehicle
