Key Responsibilities:

• Deliver intensive case management for homeless Veterans

• Attend community appointments to support individuals

• Encourage participation in community and social events

• Conduct detailed assessments to evaluate needs

• Work closely with the Housing First team on support plans

Requirements:

• Bilingual in English and French

• Degree or diploma in social or related services

• Minimum of 2 years in case management roles

• Knowledge of psychosocial rehabilitation techniques

• Full G license with a reliable vehicle
