Part-Time Social Worker

Position: Part-Time Social Worker at Bayshore HealthCare
Location: Wasaga Beach

Join Bayshore Health Care as a part-time Social Worker to support client independence and quality of life in North Simcoe Muskoka. Ideal candidates will manage caseloads and execute psychosocial treatment plans.

As part-time Social Workers, you will work 2-3 days a week, assisting clients in their homes or via virtual visits. You will communicate with families and service providers, assess clients' needs, and develop tailored treatment plans. You'll also teach coping skills and advise on community resources, ensuring clients receive comprehensive support.

Key Responsibilities:

• Collaborate with clients, families, and professionals for coordinated care

• Manage a caseload ensuring timely service delivery

• Assess psychosocial problems and track client progress

• Implement and evaluate treatment plans with community partners

• Facilitate referrals to support services in the community

Requirements:

• Two years of community-based therapy experience preferred

• Active Ontario College member in good standing

• Strong English proficiency;
French assets

• Valid driver's license and access to a vehicle

• Current CPR certification and police clearance required
