Assertive Community Treatment Case Manager

Join CMHA as a Case Manager on the Assertive Community Treatment Team, focusing on recovery-oriented mental health services in Alliston, Ontario. This role empowers clients with serious mental health issues through personalized support.

The position covers a full-time, one-year contract, requiring travel within York Region and South Simcoe. You will engage clients to assess their strengths and challenges, guiding them in setting achievable goals and accessing community resources. Evening and weekend hours may be part of the schedule, reflecting client needs in the community.

Key Responsibilities:

• Establish caseloads while adhering to Intensive Case Management standards

• Develop client-centered plans to address individual needs

• Initiate necessary referrals to internal or external services

• Address client trauma symptoms with appropriate support

• Collaborate with clients for goal setting and independence

Requirements:

• Degree or diploma in social services with preference for related field

• Registration in good standing with OCSWSSW

• Minimum one year experience in community-based settings

• Knowledge of mental health policies and community resources

• G driver’s license and standard liability insurance required
